Anyone replaced a wiper transmission? Looks easy but I was wondering if the attach bolts have nuts under the dash pad or the nuts are captive.

All of the wiper components are accessible from under the hood...no need to stand on your head under the dash!
Just spray all of the fasteners down a little beforehand with your favorite penetrating oil, and all should go nicely...
The transmission "pivots" are held on with bolts that fasten to backing plates on the back-side of the wiper compartment. As long as the nuts haven't broken loose from the backing plates, you're good to go...
